Thank you for your interest in joining the MSC team as a qualified subcontractor! MSC is looking for high-quality subcontractors for all trades related to site work, storm drainage and underground utilities.

 

1. Before filling out our qualification form, you must download and completely fill out Form W9.

 

2. You must also provide us with a copy of your GL Insurance with waiver of subrogation, Worker's Compensation, and Auto Liability certificates, naming Marietta Sand Corporation as additional insured.

3. Once you have filled out and collected the forms from steps 1 and 2, then please zip the individual PDF files into a single .zip file, with your company's name as the file name. For example, if your company's name is Bob's Concrete, then the zip file should be bobsconcrete.zip. Then, upload your zip in the form below.

 

4. To be considered, please fill out the following form. Be sure to note fields marked with an asterisk (*), as these fields are required. Some groups of questions state that all fields are required. Please be sure to take note of these instructions as well.
